Thanks Brian. I do not believe that this is a hardware nor media issue. See the bug I reported against Ubuntu 10.04 server. I tried all sorts of combinations of media, hard drives and optical drives before reporting that one. The issue occurs when trying to install from a PATA optical drive to a SATA hard drive. PATA -> PATA works. SATA -> SATA works.
