Gary, you don't have evolution-couchdb installed, since if it were
installed, it would show a message saying:
e-data-server-Message: adding type `EBookBackendCouchDBFactory'
so please install that package and try again. Closing the bug again, so
reopen if it still does not work for you, and if so, please paste the
whole output of e-addressbook-factory in the terminal
