F.Y.I. "-extension XFIXES" is supported by vnc4server_4.1.1+xorg1.0.2-0ubuntu4_i386.deb, previous version like vnc4server_4.1.1+X4.3.0-21_i386.deb does not support XFIXES.
I had all the problems listed above with vnc4server since I installed edgy, searching for the solution I upgraded to feisty with no improvement until I found that the above vnc4server pakage with extension XFIXES resolved my problem and finally I am now able to use remote connection again with KDE, I have not tested Gnome yet. This is just a work around that works fine for me and I hope it could also give some light that helps to release the final solution a.s.a.p.
