Thank you for taking the time to report this bug and helping to make
Ubuntu better.
I need two pieces of information that would be useful to have from you.
I also have two suggestions of things to try to improve the situation.
Questions:
1) How did you remove checkbox and checkbox-gtk?
I'm asking because even though removed, there appear to be some leftover
files (specifically /usr/share/checkbox/install/config).
Also, as can be seen in the log snippet below, the actual problem lies
with the debconf package which the checkbox config script is unable to
load. Your bug report didn't contain information on your version of
debconf so I'm not sure what could be the reason, but I've seen this in
systems that contain a mix of Ubuntu and Debian packages.
Here's the log file fragment:

2) Could you please post output of this command here, to check the
version of debconf installed on your system:
apt-cache policy debconf
Suggestions:
Could you please:
1) try to purge checkbox and checkbox gtk to remove all files, and try your
process again:
sudo apt-get purge checkbox checkbox-gtk
2) If that fails, or if you still see files under /usr/share/checkbox,
please manually move that directory (/usr/share/checkbox) to some other
location and retry your process. Don't delete the directory, so you have
the option of putting it back if it's determined that the problem
doesn't lie with the checkbox files themselves.
