We're seeing this particular error with one of our repositories (originally submitted via bug #656178; re-submitted in bug #815845 by myself). The usual fix was to remove the _chk_map_pyx extension, but I'd rather find a permanent fix.
So, based on your research @John, where can we go with this to try and get it fixed? Point 1) is definitely applicable in our case (~800 items).
