marzini david izaguirre molina, this issue is unreproducible via Ubuntu
11.04 (non-Unity) LibreOffice Base. I created a new database then
clicked Forms -> Create Form in Design Mode -> View -> Toolbars -> Form
Controls unchecking it made the bar disappear, checking it made it
appear. Does this work for you?
