I am on 11.10 and have this bug.
My Evince version is: 3.1.2
As Pedro Villavicencio asked to reproduce this bug:
1. Open Evince.
2. Right click Toolbar and open Toolbar Editor.
3. Try to drag *any* icon to Toolbar.
