Public bug reported:

My system menu disappears (not sure if it's a crash) rather frequently.
I was able to trigger this today by opening System Settings and clicking
Appearance and simply changing the theme. The user menu still works. I
end up having to reload Unity to bring the system menu back.

Apport doesn't appear to record a crash report for this.
