Can you post the output of the following commands (run in a terminal
window):
dpkg -L humanity-icon-theme | grep -i gtk-media-pause
find /usr/share/icons/ -name "*gtk-media-pause*"
Can you also run
sudo apt-get install --reinstall humanity-icon-theme
sudo apt-get install --reinstall gnome-icon-theme-full
Does this solve your problem?
Did you choose a special theme or are you using the default theme?
