Public bug reported:

Just installed Feisty on a new DELL D620 laptop, with 4 USB ports.
In one such port, I first plugged a SmartCard reader, which BTW I couldn't 
correctly install (I'm not sure whether this is relevant here).
After removal, and after many reboots, no USB device is detected from that port 
(it is detected from all other ports).
By using lsusb I get:

Bus 001 Device 013: ID 0b97:7761 O2 Micro, Inc. 
Bus 001 Device 012: ID 413c:a005 Dell Computer Corp. 
Bus 001 Device 001: ID 0000:0000  
Bus 002 Device 001: ID 0000:0000  

and since no smartcard reader is currently plugged in, I guess for some
reason by laptop/Ubuntu still thinks it is.
