It happens to me too on a freshly installed Oneiric (AMD64). The problem manifests itself quite consistently when I open terminal, but yesterday it happened a couple of times with Chrome too. I'm sure sometimes it doesn't do it because I was able to select "Keep in launcher" for the terminal. When it was on the launcher, I could run the terminal from there, but the small triangle wouldn't show up and clicking on the icon wouldn't give focus to the application.
