Sigh. OK, I'll put this in the "nvidia, why u no ..." bucket. Go ahead and close the bug, though seems like it would be nice to get a sensible error instead of just a corrupt screen.
(TwinView really only works with two heads. The NVS 450 is basically two video cards on one PCI card with some common support circuitry; TwinView doesn't like crossing from one GPU to the other. Xinerama may be deprecated but it's actually the only option here. If I turn it off and run nvidia-settings, it lets me combine two screens on one card, and helpfully offers to make the third screen a separate X display...)
