Right click the contact list and delete.

By the way, this bug belongs to Evolution, not to Thunderbird and
certainly not to DesktopCouch. It is Evolution that is unable to open
CouchDB contact lists and doesn't even provide fields for address,
username and password. All other contact lists from Evolution works well
in Thunderbird, which tells me that Thunderbird EDS is not to blame for
this. Google contacts configured in Evolution appears without a hitch in
Thunderbird, for instance.
