Can you do the following in a desktop that works:
1. Press the right mouse button over an empty area of the top panel. A menu
should appear, choose "Add to panel..."
2. Click on "Custom Application Launcher".
3. Type
xterm
into the Name and Command boxes. Click OK. A new icon should appear in your
panel.
4. Click it and a black terminal window should appear.
Assuming this bit works, now connect up the TV-Out and repeat your
original steps but this time try and use this new icon to launch a
terminal.
