Should be easy to reproduce, and have nothing to do with the theme in
use (I use Adwaita for both windows and controls):

1) Start with nothing opened, best just after logon, Unity 3D.
2) Open Home folder from the Unity panel, unmaximized.
3) Mouse over the minimize/maximize/close icons, they will change color/aspect 
as you mouse-hover them.
4) With only that window open, click now on the desktop anywhere to loose focus 
and keep window in sight. Buttons appearance and title bar will change, to 
visually inform that the window has lost focus (pick Nautilus because 
apparently it has the greater chances to be displayed correctly !).
5) Minimize Home window.
6) Now, you open Calculator.
7) Mouse-hover the buttons again, they will *not* change aspect, altough they 
will work.
8) Click on desktop anywhere to loose focus and keep window in sight. Buttons 
and title bar will remain in focused state. Sometimes, it happens also that it 
opens in unfocused state and always stays there (pick Calculator because is the 
most likely to have this problem, dunno why, but also happens with any other 
window, also terminal).
9) Restarting compiz-decorator has no effect at all.
10) With emerald this problem doesn't happen.
