I ran the proprietary fglrx up until about a year ago, ie before this issue. I am (and under Unity always have been) running the default Mesa/Gallium (0.4) driver.
I have thus far continuously had 100% reproducibility for the conditions I described in comment #8. My suspicion, without being very close to the associated code or looking at it yet, is that the issue is a mishandled case involving the launcher or switcher retaining focus. It may be that the dual display setup handles focus and/or workspaces differently (such as by making it one big workspace that is split in half or replicated to the different displays) and thus circumvents the normal mechanism or conditions for single displays.
