We experience the same problem in a mixed network environment. It seems that the error appears mostly (maybe only) when our Mac users edit the documents. We are using LO 3.3.2 on the Mac which connects by SMB to a Linux server. The documents are in ODT format and created with LO.
This bug is very critical for us since it leads to data loss. However, it is difficult to give a file that reproduces the problem since you never know which image will disappear.
