Confirmed it is a duplicate of bug 902491: changing the command in
/lib/udev/rules.d/85-lvm2.rules to:
"watershed sh -c '/sbin/lvm vgscan --noudevsync; /sbin/lvm vgchange
--noudevsync -a y'"
and running "update-initramfs -u" fixed the problem.
