I have been trying to reproduce this using a Precise Xen host (Xen 4.1.2) and a 
CentOS 5.6 based host (Xen 3.4.3). Both do not seem to have those issues. I am 
not sure how often this happens to you.
One thing to try would be to add barrier=0 to the mount options of /. I think I 
remember having read something about barrier requests causing problems. Just 
not sure this is or was related to dom0 or not, but maybe it is.
