If I'm to believe the behaviour on my system this appears to be relevant:

path="/org/bluez/19690/hci0/dev_00_16_FE_D6_F8_84""): initialization failed.

>From testing, it seems like this is because by default, bluez upstream
has moved from the older socket-based API to a DBus API using the
/MediaEndpoint paths. Pulseaudio doesn't look like it's able to handle
this yet, and as soon as I enable the socket API things start to work
again.
