I'm not entirely certain if this was the actual fix to this multi-head issue for me (nvidia, twinview, 2nd head on the right rotated and panning, nvidia propritary driver, lucid), as this was a quite a while ago.
However the most significant changes I did back then, was switching to the xorg-edgers-ppa and ubuntu-x-swat-x-updates ppas and updating *all* apps.
