I installed Onieric (as opposed to in-development Precise) on my mother's new laptop today, and realised what the problem really was; the volume control on the Unity login screen is not synchronised with the sound indicator on the desktop. On bootup, the login screen's volume control is always at full, which Pulseaudio is obviously responding to. The state is not updated until I adjust the indicator's volume control - hence the problem. I think.
