I think this problem _may_ be a result of unity incorrectly handling existing windows when compiz crashes - all the windows in my screenshot above relate to applications I start "immediately" on login (via ~/.config/autostart/*.desktop). I think compiz may have crashed earlier and on restart duplicated the list of already running application windows. Skype not being doubled up might be explained by the fact that it is normally "docked". Firefox, I'm not sure about, but I do have a lot of tabs open so that might be down to a timing issue.
