This is a dual monitor problem.
It  can cause unity to die (even the mouse disappears) but the [Ctrl]+[F1] 
still works
changing the monitors available can cause all 3 problems:
1) menu bar is drawn to a disabled monitor not the primary monitor 
2) all windows get a max width or height of 20px
3) unity crashes
