This could be a bluez issue. To make sure, could you get bluez debug
logs?

You can do this easily:

sudo stop bluetooth
sudo /usr/sbin/bluetoothd -n -d

Then attach the output of bluetoothd, you should be able to see whether
it tries to hand off the device to pulseaudio. If it does, then
pulseaudio debug logs will be needed as well.
