I can't find anything wrong by looking at these settings. I've been
grepping through the source code to see if there are any changes in the
codec driver code that could be responsible, but could not find any.

Can we rule out that this is a hardware error, e g by booting up a 11.10
live-CD, verify that the headphone still works, then submitting an alsa-
info (see https://wiki.ubuntu.com/Audio/AlsaInfo ) in that working
state?
