To figure this one out, we are going to need verbose logs from
pulseaudio, to see what errors it shows. However, doing so is a little
tricky, as you cannot simply restart pulseaudio with logging on, since
the new instance will resolve the problem.
one approach would be to add "set-log-level 4" in /etc/pulse/default.pa, above
"load-module module-udev-detect". Then reboot and verify that the problem is
still present.
Then grep your syslog for interesting messages from pulseaudio.
