This exact thing happened today with an up-to-date ubuntu 12.04 installation, this time with Chromium. However, I am still not sure exactly how to reproduce this.
I did however notice one thing. When the first Chromium window closed, I clicked the icon to start a new one. When I alt-tabbed, that one closed as well. So I started another one. I repeated this a couple of times. I then forced a lightdm restart by pressing ctrl+alt+1 and running sudo service lightdm restart. When the restart happened a bunch of chromium windows flickered by, closing as the force quit was taking effect. It seems like the applications actually never crashed, thus not triggering apport to create a report. Instead, they seem to have been lurking around outside visible space. So, it seems more like Unity is to blame than any one application. If I figure out a sure way to reproduce the steps - I will get back.
