I can reliably reproduce this bug on a fresh, stock install of 12.04 with all updates done. Install, say, Medit. It appears in the taskbar. Unintall Medit, icon is still in the taskbar.
It's amazing to me that such an *obvious* bug could make it into a final LTS release. It's the sort of easily-encountered, semi-cosmetic breakage that turns people off immediately -- when the system works in unexpected ways, the motivation to use it drops.
