Public bug reported:

Upgraded a system running 11.10 to 12.04. During the upgrade when it
went to reinstall grub on the boot sector the upgrade got stuck and
eventually dpkg aborted. Looked in dmesg and the issues seen in bug
#550559 were occuring, /dev/sda (the OS) and /dev/sdb (some data) were
no longer accessible. System was unusable so I power cycled. Grub was
not fully installed so it would not boot. I've booted off of the 12.04
live USB and installed boot-repair. Ran boot-repair and it caused the
same issues with the drives becoming unaccessible. So it appears that my
system is toast with 12.04 at least. I am going to try a 11.10 live USB
and see if I can make any progress there.
