I have this same issue (items 1, 2, and 3 above) on linux- image-2.6-20-16-lowlatency (I didn't spend any time on -generic). For fun, I copied the snd-hda-codec.ko and snd-hda-intel.ko modules from 2.6.20-15-lowlatency into 2.6.20-16-lowlatency, and low and behold, sound worked.
