Sorry about the wait. I've finally re-upgraded to 12.04, reproduced the bug and ran the command to report the issue. On the first run my monitor was detected correctly and I was able to operate without a hard crash. After restarting the laptop, I was able to login fine again. I then entered display settings and changed the monitors to be placed correctly (i.e. moved the main monitor to the left and the laptop to the right) at which point the OS froze after hitting Apply. I rebooted with the HDMI still plugged in and once again after the login screen I ended up with a lot of weird colors frozen on the screen and had to power off by holding the power button. I unplugged the HDMI and was able to boot into Ubuntu successfully to file this bug report. Let me know if there's anything else i can do to be of assistance in resolving this.
