This seems to be related to the following upstream bugs:

The general trend seems to be the following:
- KVM is used
- ksmd is the current comm
- a GPF is triggered due to the upper 16 bits of a register used as an address 
being cleared

It would be extremely helpful to test with KSM disabled, to see if this
is the root cause.
