I've got the same issue in Precise; sometimes the network disconnects, and nm-applet asks for the password (even though it's stored) every few minutes. Unfortunately if a password prompt is already shown, nm-applet periodically issues a new one; so, if this happens while I'm not at the PC, when I get back to the PC I find tons of them.
Also, even I confirm the dialog (or if I re-type the correct password), it doesn't reconnect. I'm attaching the syslog (look for the PID 915).
