I have the same problem with a Genius NetScroll 200 Laser USB mouse. The mouse does not use the kernel driver "usbmouse.ko" (it is never loaded) The problem occurs when PC resume from sleep to RAM in 95% of the time. Before sleep to RAM, these are the outputs of lsub, xinput and dmesg: $lsusb Bus 003 Device 012: ID 0458:003a KYE Systems Corp. When PC wake up from sleep to RAM the mouse is not detected anymore and lsusb/xinput stop to display information about the mouse. The problem only occurs with the mouse. I have other devices like USB graphics tablet and it works fine after resume. I try several Kernels on Kubuntu 64bits 11.10 Oneiric and 12.04 Precise, and the problem always happens.
