Hi,

I have the same symptoms as described in this bug.

I have updated my system from 10.10 to 12.04 and its after that I got this 
problem.
Pulseaudio does not start, and manual 'pulseaudio' prints 'Killed' at the end 
(along with 'module-combine is deprecated').
I tried to purge and reinstall pulseaudio (and all packages that depends on it, 
but the problem remains).

The problem is originating from 'module-combine' which is enabled via 
"Simultaneous Output" under 'paprefs'.
After disabling that pulseaudio starts on my system and works as it should.

('sudo pulseaudio --system' also worked, and compairing the logs from "normal" 
and "system" was what led me to the 'paprefs' setting).
(I also had working sound when using ALSA directly).

So the problem seems to be with 'paprefs' using 'module-combine' instead of 
'module-combine-sink', or something in 'module-combine-sink' as that is loaded 
instead by pulseaudio.
I dont know why and have not investigated the problem further, but if I enable 
the "Simultaneous Output" in 'paprefs' the problem is back.


With "Simultaneous Output" enabled:

$ pulseaudio
W: [pulseaudio] pid.c: Stale PID file, overwriting.
W: [pulseaudio] sink.c: Default and alternate sample rates are the same.
W: [pulseaudio] source.c: Default and alternate sample rates are the same.
W: [pulseaudio] module.c: module-combine is deprecated: Please use 
module-combine-sink instead of module-combine!
W: [pulseaudio] module-combine.c: We will now load module-combine-sink. Please 
make sure to remove module-combine from your configuration.

With "Simultaneous Output" disabled:

$ pulseaudio
W: [pulseaudio] pid.c: Stale PID file, overwriting.
W: [pulseaudio] sink.c: Default and alternate sample rates are the same.
W: [pulseaudio] source.c: Default and alternate sample rates are the same.
