This problem occurs frequently on my DELL 6500 on startup but not all the time. I have submitted multiple reports using the automatic error report generator.
This page notes only 4 people are affected but even a cursory google shows many more than that. Apparently the bug has been arround since at least Sep 2011 (Ubuntu 11.10).
