This bug is a regression starting with Ubuntu 11.10. In Ubuntu 11.04
everything worked just fine. I believe this bug is actually a pulse
audio bug. PA doesn't change the alsa volume which is at maximum. The
desktop manager uses the alsa volume, this is why it's at maximum at
login. The problem is, that PA only applies it's volume after the user
changes it.
