I see the same problem, behaves somewhat differently on different
machines/ports. I use two devices connected via two Logitech Unifying
dongles on each of these computers, running latest 12.04.

On my desktop computer (Zotac amd64) I often need to unplug and replug
(once) one or both devices when X starts. Then they work fine. This only
started to happen after upgrade to 12.04.

On Dell Latitude E 6510 both work fine most of the time, sometime I need
to unplug, plug them.

On Zareason Gazelle Pro notebook (Intel ia7) which has three USB ports -
one high speed USB 3 and two slower USB 2 ports the device plugged in
the USB 3 port always works, the device plugged in any of the slower
ports usually does not, if I unplug and plug in again enough times it
eventually starts working.

When the device is not working I see same messages in kern.log:

Not sure if it's relevant or not but the keyboards work fine during boot
on all machines in any port.
