I have thought about the reports and the test results and the test procedures we have run until now. Trying to help, I have repeated the v2-v3 process of disinstallation and reinstallation. I have obtained the same results: if the problem is random, it's... repeatably random!!! :) No, ok, it's unlikely a random malfunction.
I am attaching a new version of the init script. The difference from the previous version, attached to comment #35, is that this version has been obtained on a system that never tried to boot v3 of kernel. I.e., the init script ought to be almost identical to the version of comment 35, because it's obtained from the same debian package, of v3 kernel. If it's not, the boot process corrupts (or we'd better say, fixes) the init script. Sorry, I don't remember the file compare command... but I presume you can easily check this out.
