I can confirm that this problem exists with Ubuntu 12.04 up to date as of today. I don't recall it happening right after upgrading from 11.10, but after one of the subsequent updates. Unfortunatelly, I neglected the most important item of noting after which update it occurred. Without being exact, my Logitech Wireless Mouse is dead about 30-50% of the time after a cold boot. Unplugging the USB receiver and leaving it out for about 10-15 seconds and then plugging it back in corrects the problem about 80% of the time. I searched for this bug report because this is such an annoying bug. I will attempt to capture the information requested of the bug reporter and add it as a confirmation of the bug.
