Looks like you are missing the gtk-media-pause icon, can you post the output of
the following command in a terminal:
find /usr/share/icons/ -name "*gtk-media-pause*"
apt-cache policy gnome-icon-theme-full humanity-icon-theme
Normally these icon themes are present on a default Ubuntu installation,
you can try reinstalling:
sudo apt-get install --reinstall humanity-icon-theme
sudo apt-get install --reinstall gnome-icon-theme-full
Does this solve your problem?
