I had very similar issues but my problem was /lib/udev/rc_keymaps/ was not proper for my remote. I was using an Antec Veris case that depends on the imon driver.
With lirc configured to use the devinput I could get some buttons but not all of them via 'irw'. My solution was to take the older /usr/share/lirc/remotes/imon/lircd.conf.imon-pad and merge in into the new /lib/udev/rc_keymaps/imon_pad to have all of the codes I use to have prior to a 12.04 dist upgrade. Once this was done and I rebooted I could get all my keys. (more notes on my fix here: http://torawk.blogspot.ca/2012/08/devinput-antec-veris-imon-remote- used.html ) I came across this bug while trying to fix it and it sounds like similar symptoms -- so perhaps this is an alternative fix? and easier to do than recompiling.
