I've experienced this bug at least twice after upgrading my ThinkPad
T420 (also with Quadro NVS 4200M graphics) from Kubuntu 11.10 to 12.04
LTS about a week ago.

I also recall seeing a log similar to that in the last bullet point in
your initial message being posted every time I open the lid when the
problem doesn't occur.

If I recall correctly, when the problem occurs and I try to log back in,
I can't even get to the desktop.  The login process just hangs right
before the desktop would come up, forcing a hard reboot (since I could
also not access any other text terminal)

Seeing the follow lines reference nouveau had puzzled me:

I would have assumed that the lack of Optimus support would have meant that 
drivers for the NVIDIA card would not be installed, since only the Intel 
Integrated Graphics can be accessed, at least under my current BIOS 
configuration (I don't recall ever installing them myself, and the integrated 
graphics have been powerful enough for my needs in Linux).  Searching in the 
package manager for "nouveau" reveals that I have the following packages 
installed:
xserver-xorg-video-nouveau
libdrm-nouveau1a
libdrm-nouveau1a (i386)

Judging by the posted Xorg log's (and my Xorg log's) references to the
Intel Integrated Graphics and lack of references to the NVIDIA card ,
I'd say that we both have Optimus and that the operating system is just
using the integrated graphics.  Interesting that the NVIDIA card appears
in both our lspci outputs, though.  Perhaps that explains why the
drivers were installed.

Therefore, my suspicion is that this problem is somehow nouveau and
Optimus related.  I'm going to try uninstalling the nouveau packages
mentioned above and see whether that causes the problem to go away.
