RESOLVED: The problem is caused by a default selection which changed at some point in Firefox releases. In Firefox File==>Page Setup there is an option:
"Format For" The default is "Any Printer for Portable Documents". If this is changed to a specific printer, then the problem goes away. Apparently, the formatting for "Any Printer for Portable Documents" assumes that the printer can print from the very top of the page to the very bottom which is not true of Laser Printers.
