Figured it out.
Even though pulseaudio-module-bluetooth is installed, it is not
configured.
module-bluetooth-discover is NOT activated automatically.
This must be added to /etc/pulse/system.pa:
### Automatically detect bluetooth devices
load-module module-bluetooth-discover
This should be configured automatically if pulseaudio-module-bluetooth
is installed and a bluetooth tranciever is available.

** No longer affects: bluez (Ubuntu)
** No longer affects: gnome-bluetooth
