On Ubuntu 12.04 I get this problem with Writer, Impress and Calc. There is an additional case when no icon at all appears in the launcher.
As workaround I didn't open the files through Nautilus: I opened the application first and then used the File > Open option. I agree with comment #39. As for comment #76, installing the ppa solved the problem, although I still can see the white icon appearing for a brief moment before the proper one.
