Same problem here with fairly default settings.
OS: Ubuntu 12.10 64bit upgraded from Precise
Hardware: Core i5-3360M IvyBridge, current Intel drivers

Inactive windows for Nautilus look OK (slighly darker font compared to
active, as by design), but Firefox and others consistently get a white
titlebar background as shown in the screenshot attached by the original
reporter. For me, they don't correct themselves or change in any way
over time.

Applications looking good: Nautilus, Evince, System Monitor, LibreOffice, most 
Accessoires entries (calc etc), ...
Some programs with white inactive bg: Firefox, Thunderbird, gnome-terminal, 
Audacious, Mahjongg, Transmission, Geany, etc.

I'm beginning to suspect that it's simply a theming issue, but why the
weird split across applications?
