libreoffice-writer 1:3.5.4-0ubuntu1.1

In LibreOffice Writer I create headings in my document, then go to the
menu item "Insert >  Indexes and Tables - Indexes and Tables" (as per
http://help.libreoffice.org/Writer/Creating_a_Table_of_Contents ). I
expect the options dialog to appear, but instead the dialog window seems
to hang for a while and then the program crashes. Recovering the
document works fine but no TOC was added.

Output from running "soffice toc-test.odt" is attached.
