** Description changed:

  Currently when adding new keyboard shortcut:
  
  * there is no hint when the shortcut should be provided, the window
  simply says "Command Shortcut" (see screen) so it could be understood as
  a summary/confirmation message
  
  * if shortcut is not provided when window expects it, item is simply not
  added at all, providing no useful feedback
  
  This is quite frustrating for user not familiar with how the keyboard
  manager works, it seems as if something was broken but it's not clear
  what.

+  0 - Have Ubuntu (12.04 LTS in my case) with Xfce4 installed
+  1 - Login to an Xfce session
+  2 - Open Keyboard Settings Manager
+      - "Aplications Menu" -> "Settings" -> "Keyboard"
+      - or run `xfce4-keyboard-settings`
+  3 - Switch to "Application Shortcuts"
+  4 - Click "Add"
+  5 - Fill out something (does not need to be a working command--any string 
will do)
+      to the "Command" field
+  6 - Click "OK"
+    > result: a small window appears, expecting shortcut, which is not apparent
+    > oracle: it should be apparent that shortcut definition is expected
